Fosbu Ingredients and Usage

The active ingredient is Sofosbuvir, a nucleotide analog inhibitor of HCV NS5B RNA-dependent RNA polymerase. One 400mg tablet is taken once daily with or without food, always in combination with other antiviral agents as per prescribed regimen. Fosbu is clinically indicated for:

• Chronic Hepatitis C virus (HCV) genotype 1, 2, 3, and 4 infections
• Treatment-naïve and treatment-experienced HCV patients
• Patients with compensated cirrhosis
• HIV-HCV co-infected patients
• Post-liver transplant HCV recurrence prevention

How Does Fosbu Work?

Fosbu provides potent antiviral activity through targeted polymerase inhibition:

1. NS5B Polymerase Inhibition: Acts as a defective substrate for HCV NS5B RNA-dependent RNA polymerase
2. Chain Termination: Incorporates into viral RNA causing premature chain termination
3. Viral Replication Blockade: Suppresses HCV replication in infected liver cells

Fosbu Side Effects and Warnings

Common side effects include fatigue, headache, nausea, insomnia, anemia, and rash. Most adverse effects are mild to moderate in severity.

Serious Considerations

1. Hepatitis B Reactivation: Risk of HBV reactivation in co-infected patients - test for HBV before starting
2. Bradycardia with Amiodarone: Serious symptomatic bradycardia may occur with concomitant amiodarone
3. Drug Interactions: Significant interactions with rifampin, carbamazepine, and St. John's wort
4. Renal Impairment: Safety not established in severe renal impairment (eGFR <30 mL/min)
5. Pregnancy: Use only if potential benefit justifies potential risk
